What Is NAD+?

NAD+ is a naturally occurring molecule that functions as a coenzyme in hundreds of biochemical reactions. One of its best established roles is transferring electrons during cellular metabolism, helping cells convert nutrients into usable energy.

NAD+ is also consumed by enzymes involved in DNA repair, cellular signaling, stress responses, and regulation of gene activity. Because NAD biology changes with metabolic state, disease, and aging, strategies intended to influence NAD+ availability have become an active area of research.

NAD+ is not a peptide. It is a cellular coenzyme, which is why New Horizon categorizes it as a supportive therapy rather than a peptide therapy.

These biological roles are well established. However, the fact that NAD+ is essential to cellular metabolism does not by itself establish that supplemental or injectable NAD+ improves energy, cognition, recovery, longevity, or other wellness outcomes.

What Does the Research Show?

NAD+ biology is supported by an extensive scientific literature, but evidence for NAD+ supplementation depends heavily on the compound and route being studied. Human trials have more commonly evaluated NAD+ precursors such as nicotinamide riboside (NR) and nicotinamide mononucleotide (NMN) than injectable NAD+ itself.

A 2026 systematic review identified 33 human intervention studies involving NAD-related compounds. Oral NR and NMN consistently changed NAD-related biomarkers, but effects on metabolic, vascular, functional, and other healthspan outcomes were mixed or often limited to specific endpoints.

Importantly, that review found no eligible outcomes trials of intravenous or intramuscular NAD+ itself for anti-aging or wellness. A separate 2026 retrospective IV pilot provided preliminary tolerability information but was not designed to establish anti-aging or wellness effectiveness.

What to Know

Injectable NAD+ used in compounded preparations is not an FDA-approved drug product for anti-aging, energy enhancement, cognitive enhancement, recovery, or general wellness.

FDA has specifically warned sterile compounders to use NAD+ ingredients suitable for sterile drug compounding. The agency reported adverse events after injectable NAD+ products, including severe chills, shaking, vomiting, and fatigue; some reports were consistent with excessive endotoxin exposure from unsuitable ingredients.

NAD is currently listed by FDA as a Category 1 bulk substance under evaluation for use in compounding under section 503A. Category 1 status is not FDA approval and does not establish safety or effectiveness for a particular clinical use.

A provider should review health history, medications, treatment goals, formulation, route of administration, and available alternatives before compounded NAD+ therapy is considered.
